We use the Athena app for your official medical records, lab results, and care summaries. Please Note: You will receive a separate email from "AthenaHealth" within 24 hours to create your password.

03 / Complete Online Check-In & Schedule Your Initial Health Assessment (IHA)
Our online check-in allows you to complete intake forms and share relevant medical information in advance. After check-in, you can schedule your Initial Health Assessment.
